Are you at risk for Type 2 Diabetes?

FMS Pharmacy is committed to helping you or a loved one decrease the chance or stay away from the harmful effects of Type 2 Diabetes.

FMS Pharmacy is a recognized site by the American Association of Diabetes Educators (AADE) to provide the Centers for Disease Control and Prevention (CDC) led National Diabetes Prevention Program. This evidenced based program has been proven to help you or a loved one decrease your risk or delay the onset of type 2 diabetes by up to 58% through lifestyle changes of weight loss and healthy eating.

Our pharmacists are trained Lifestyle Coaches through AADE to provide this program to you.

This program consists of 16 core classes delivered in the span of 24 weeks followed by 6-8 post core classes that are delivered monthly to finish out the year long course.

Participate in the National Diabetes Prevention Program (DPP) offered by FMS Pharmacy based on the criteria below.

• 18 years or older BMI ≥24 kg/m2 (≥22 if Asian)

• Diagnosis of pre-diabetes or Gestational Diabetes Mellitus (GDM) based on one or more of the following below:

→ Fasting blood glucose (range 100–125 mg/dl)

→ 2-hour glucose (range 140–199 mg/dl)

→ HbA1c (range 5.7–6.4)

→ Previous GDM (may be self-reported)

→ Paper risk test (performed at pharmacy)
